Eligibility

Inclusion criteria

Inclusion criteria: 1. 65 years of age or older; 2. Using at least five medicines; 3. Using at least one medicine from ATC group A or B; 4. Being non-adherent to at least one medicine.

Exclusion criteria

Exclusion criteria: 1. Resident in nursing home; 2. Life expectancy of less than 3 months; 3. No informed consent.
