Inclusion criteria

Inclusion criteria: Patients with diabetes mellitus and comorbidity participating in the diabetes management programme provided by the Diabetes Research Centre in Hoorn (NL).

Exclusion criteria

Exclusion criteria: A health problem probably leading to death within 1 year.
