Inclusion criteria

Inclusion criteria: Elderly people, who are admitted to a geriatric rehabilitation department in a nursing home (after hospital intervention) for rehabilitation care.

Exclusion criteria

Exclusion criteria: Patients with a preexisted dementia syndrome.

1. Which types of integrated care programs at individual client level will be generated from the pilot organizations? 2. What is the opinion of patients and there caregivers about these programs? 3. Which effects do these programs have on patient and caregiver outcomes?

Secondary

MeasureTime frame
A coherent somatic rehabilitation care program in the region with added value for patients.
