Eligibility

Inclusion criteria

Inclusion criteria: Aged 70 years or older; receiving medication from the public pharmacy; use of 5 or more different medications; chronic use of antidiabetic (at least 1 medication next to metformin) and/or cardiovascular medication; HbA1C < 54 mmol/mol; systolic blood pressure < 140 mmHg.

Exclusion criteria

Exclusion criteria: Not having signed the statement of consent; unable to fill out questionnaires; not able to speak, write, or understand the Dutch language; life expectancy < 3 months; participation in other research projects.

Design outcomes

Primary

MeasureTime frame
The primary outcome is deprescribing and will be defined as the percentage of reduction of medication in the intervention group compared to the control group.

Secondary

MeasureTime frame
1) Number and type of the change in prescribed medications, results, and perstistency; 2) Satisfaction level of the patients (TSQM and PREM questionnaires); 3) Satisfaction level of the health care providers.
