Eligibility

Inclusion criteria

Inclusion criteria: The patients are: - 18 years or older. - competent to make decisions concerning their health - Have =2 out of the 3 following problems: psychiatric, chronic disease or social problem. Have more than average acute medical care usage ( =2 times per year).

Exclusion criteria

Exclusion criteria: The exclusion criteria done by the general practitioner: • The patient is terminal. • The patient has dementia and is unable to communicate effectively. • the patient is lacking decision making capacity (concerning their health). • The patient doesn’t give permission to participate in the study.

Effects on hotspotters’ acute medical care use in society.

During the study data from various health related questionnaires will be collected on three timepoints (0, 3 and 6 months).
