Primary objectives are to describe the frequency and type of workarounds in the BCMA process and to determine the association of workarounds with medication (administration) errors in the BCMA process. Secondary objective is to explore the potential risk factors (determinants) for workarounds.

Inclusion criteria
Inclusion criteria: Hospitalized patients receiving medication on wards that participate in this study
Exclusion criteria
Exclusion criteria: Hospitalized patients younger than 18 years
