Eligibility

Inclusion criteria

Inclusion criteria: 1. Persons, 18 years and older; 2. The person has a physical disability and/or chronic disease; 3. The person receives inpatient or outpatient care within one of the participating rehabiltation centers or hospitals; 4. The person participates in the program "Revalidatie, Sport en Bewegen".

Exclusion criteria

Exclusion criteria: The person is not able to complete the questionnaires, even with help.
