Inclusion criteria

Inclusion criteria: - Patients with chronic diseases, which are contacted by SGF (Samenwerkende Gezondheid Fondsen, i.e. Hartstichting, Longfonds, Diabetes Fonds, Spierfonds, ReumaNederland, MLDS, Hersenstichting, Nierstichting, MIND). - Minimum age of 18 years

Exclusion criteria

Exclusion criteria: - Not being able to read and understand the Dutch language - Unable to provide consent due to incapacity
