Eligibility

Inclusion criteria

Inclusion criteria: 1. Patients older than 60 years; 2. using five or more prescription-only chronically used drugs when admitted to the hospital; 3. discharged from the departments of cardiovasculair disease, lung disease or internal medicine; 4. patients have to give written consent.

Exclusion criteria

Exclusion criteria: 1. Patients discharged to a nursing home; 2. patients to confused to beable to adequately participate in the intervention will be excluded; 3. terminal ill patients; 4. patients unable to cummunicate in the Dutch language.

Design outcomes

Primary

MeasureTime frame
Effects will be assesed in terms of changes in the occurence of drug related probems.

Secondary

MeasureTime frame
Secondary endpoints are attitude to drugs, persistence of drug adherence, quality of life, re-hospitalisation related to medicines and cost-effectiveness.
