Eligibility

Inclusion criteria

Inclusion criteria: 1. General practices and nephrology practices; 2. Patients older than 18 years.

Exclusion criteria

Exclusion criteria: 1. Practices that already use Telenephrology; 2. Patients with acute nephrologic disease, that need referral the same day; 3. Patients unable to give informed consent for the use of Telenephrology.

MeasureTime frame
Number of referrals to nephrologist care.

1. Number of consultations of nephrologists; 2. ICPC coding of patients with CKD; 3. Number and height of known blood pressures in patients with CKD; 4. Metabolic parameters in patients with CKD; 5. Newly found patients with CKD; 6. Number of consultations and referrals in newly found patients with CKD; 7. Costs and profits of Telenephrology.
