Eligibility

Inclusion criteria

Inclusion criteria: Patients aged 18-75 years who are enlisted with three general practices (1) Polypharmacy patients using five or more different types of medication , (2) Parents of children younger than four years of age, (3) “Other patients”, patients who do not belong to the first two groups

Exclusion criteria

Exclusion criteria: Severe psychiatric and cognitive problems, language problems. By GP expected harm for patient or others from having access to records in general practice.

MeasureTime frame
Uptake of intervention, barriers and facilitators for using the intervention, characteristics of users and non-users of the intervention Effects: patient satisfaction, utilisation of GP services, outcomes related to patient involvement including taking an actve role in decision-making medication adherence.

Usability and usefulness of the intervention for patients attitudes, opinions, experiences of GP staff
