Inclusion criteria
Inclusion criteria: 1. Persons aged 65 years or older; 2. Prescribed 5 or more regular medicines, including at least one cardiovascular or antidiabetic drug
Exclusion criteria
Exclusion criteria: 1. Persons receiving repeat prescriptions solely from a specialist
