Eligibility

Inclusion criteria

Inclusion criteria: 1) Patients with a (ischemic or hemorrhagic) stroke that have to make a decision regarding discharge location and type of care after discharge from the hospital; 2) Age = 18 years; 3) Able to understand the Dutch language in speech and writing.

Exclusion criteria

Exclusion criteria: 1) Dementia (based on clinical assessment); 2) Reduced consciousness (based on clinical assessment).
