Eligibility
Inclusion criteria
Inclusion criteria: - > 65 years old - Use of 5 or more chronic, systemic drugs (anticancer drugs excluded) - Be able to read and understand the Dutch language - Having completed at least two cycles of anticancer treatment

Exclusion criteria: There will be no specific exclusion criteria in this study.

| The main objective of this study is to compare the patients' satisfaction with their medication before and after performing the polypharmacy assessment (i.e. at baseline vs at follow-up moment of one month after the polypharmacy treatment plan is discussed with the patient). This satisfaction wilt be measured with the 'Treatment Satisfaction Questionnaire for Medication' (TSQM) | — |

| Additionally, in order to assess if a possible difference in TSQM is clinically relevant, the minimal clinically important difference (MCID) wilt be determined, both through an anchor-based and distribution-based method. The `patients global impression of change' (PGIC) scale wilt be used as anchor. Other secondary objectives are measurements of the change in number of potential drug-related problems (DRPs) and the pill burden before and after the polypharmacy assessment. In addition, the number and type of pharmaceutical recommendations and implementation rate wilt be measured. Finally, a concise overview of the costs will be given. | — |
