Brief summary
The goal of this clinical trial is to learn the effect of self prepare by multimedia program before going to heart surgery. The main questions it aim to answer is: Is preparing for heart surgery on your own or with a physical therapist the same? Researchers were compare the effects of preoperative physical therapy between multimedia and traditional programs in patients undergoing open heart surgery. Participants were received traditional therapy from a physical therapist or used a multimedia self-learning program.
Detailed description
This randomized controlled trial compared the effects of multimedia and traditional preoperative physical therapy programs in 70 patients undergoing elective open-heart surgery at Khon Kaen University, Thailand. Patients were divided equally into control and experimental groups. The control group received traditional therapy from a physical therapist, while the experimental group used a multimedia self-learning program. Both groups also received a leaflet with preoperative guidelines. After completing the programs, patients were assessed on chest expansion and surgery knowledge. Both groups showed significant improvement in upper and lower chest expansion after the programs. However, there were no significant differences between the groups in chest expansion or knowledge scores. The study concluded that a preoperative multimedia self-learning physical therapy program is as clinically effective as traditional therapy for patients preparing for open-heart surgery.
Interventions
A two-dimensional (2D) animated video was developed to deliver educational content regarding open-heart surgery procedures (sternotomy and general anesthesia), the preoperative physical therapy program (diaphragmatic breathing, lower-lobe breathing, and chest expansion exercises), potential postoperative complications (e.g., pneumonia and atelectasis), therapeutic exercises (ankle pumping), and mobility training (bed mobility and transitioning from lying to a sitting position)

Intervention model description
The control groups received a traditional preoperative program from an experienced physical therapist and the experimental groups received a preoperative self-learning with multimedia program. All the patients in both groups also received the leaflet about preoperative program guidelines.
Eligibility
Inclusion criteria
* Patients who undergoing elective open-heart surgery at the Queen Sirikit Heart Center of the Northeast, Faculty of Medicine, Khon Kaen University, Thailand * Age 18-70 years old * No lung atelectasis * No communication limit
Exclusion criteria
* Lung diseases such as COPD, Asthme * Hearing loss, blind and learning problem
