Brief summary
This prospective observational study will evaluate the association between ERAS protocol compliance and postoperative nutritional outcomes in 120 patients undergoing lung cancer surgery. ERAS compliance will be recorded, and patients will be classified as having high, moderate, or low compliance. NRS-2002 and PNI will be assessed before surgery and at postoperative 48 hours. Hospital stay and postoperative complications will also be evaluated.
Detailed description
This single-center observational cohort study will include 120 patients aged 18-85 years undergoing elective lung cancer surgery. No intervention will be assigned. ERAS compliance will be categorized as high (≥80%), moderate (60-79%), or low (\<60%). Changes in NRS-2002 and PNI, hospital stay, and postoperative complications will be evaluated using routine clinical data.
Interventions
Patients receive routine perioperative care according to the institutional ERAS protocol. No intervention is assigned by the investigators. ERAS compliance is assessed observationally.

Design outcomes
Primary
| Measure | Time frame | Description |
|---|---|---|
| Change in Nutritional Risk Screening 2002 Score | From preoperative assessment to 48 hours after surgery | NRS-2002 will be assessed before surgery and at postoperative 48 hours. The score ranges from 0 to 7, with higher scores indicating greater nutritional risk. Change will be calculated as the postoperative score minus the preoperative score. |
Secondary
| Measure | Time frame | Description |
|---|---|---|
| Change in Prognostic Nutritional Index | From preoperative assessment to 48 hours after surgery | PNI will be calculated as (10 × serum albumin \[g/dL\]) + (0.005 × total lymphocyte count \[/mm³\]). Higher values indicate better nutritional status. Change will be calculated as the postoperative value minus the preoperative value. |
| Length of Hospital Stay | From surgery to hospital discharge, assessed up to 30 days | Number of days from the date of surgery to hospital discharge. |
| Postoperative Complications | From surgery to 30 days after surgery | Occurrence of any postoperative complication documented during hospitalization or at the 30-day follow-up. |
| ERAS Protocol Compliance | Perioperative/Periprocedural | The number of applied ERAS components divided by the total number of applicable components and expressed as a percentage. Higher percentages indicate greater compliance. |
