Inclusion criteria

* Hospitalized patients aged at least 65 years * Taking at least six prescribed medicines regularly, including at least one potential inappropriate medication

Exclusion criteria

* Patients who refused informed consent * Discharged before consent could be obtained * Cognitive impaired
