Inclusion criteria
Inclusion criteria: Participants were inpatients =18 years discharged on an oral anticoagulant for treatment
Exclusion criteria
Exclusion criteria: Excluded were those with severe cognitive impairment, inability to communicate or to be followed-up, discharged on an anticoagulant for prophylaxis.
