Inclusion criteria
Inclusion criteria: 1. Individuals aged 65 years or older 2. Individuals living in the community within urban–rural areas 3. Individuals who receive an explanation of the study purpose and procedures and voluntarily provide written informed consent 4. Individuals who are able to participate in baseline health assessment and follow-up repeated-measure data collection 5. Individuals who are able to communicate and understand the researcher’s explanation 6. Individuals who agree to dashboard registration and the collection and management of health vulnerability screening data and repeated-measure data
Exclusion criteria
Exclusion criteria: 1. Individuals who are judged to have difficulty understanding the study purpose and procedures or providing voluntary consent 2. Individuals who have difficulty communicating or completing the assessment due to impaired consciousness, severe cognitive impairment, or severe hearing or speech impairment 3. Individuals who require immediate treatment or hospitalization due to acute illness or worsening health status 4. Individuals who are judged to have difficulty being contacted or participating in follow-up repeated-measure data collection 5. Individuals who do not consent to participate or who withdraw their intention to participate during the study
