Inclusion criteria
Inclusion criteria: The study includes high-risk older adults aged 65 years and older who are discharged from Seoul Metropolitan Government–Seoul National University Boramae Medical Center, regardless of sex.
Exclusion criteria
Exclusion criteria: Participants meeting any of the following criteria will be excluded: 1. Patients who do not wish to receive the discharge planning service 2. Patients transferred to another hospital due to the need for continued inpatient treatment after discharge 3. Patients discharged to long-term care facilities or nursing homes rather than home 4. Patients with dementia or those who are unable to communicate 5. Patients who are difficult to manage within the care system due to special medical needs
