Inclusion criteria

Inclusion criteria: Adults aged 19 years or older who visit the Heart Failure Clinic as new or returning patients and provide informed consent for the study

Exclusion criteria

Exclusion criteria: Individuals who are unable to receive the pre-visit questionnaire message sent via smartphone or who cannot comprehend and provide appropriate responses to the questionnaire message
