Inclusion criteria
Inclusion criteria: ? Older adults (1) Aged 65 years or older (2) Frailty score = 1 based on the frailty assessment tool (3) Able to communicate (4) Agreed to participate in the study ? Coordinators (visiting nurses or community-based healthcare providers) (1) Healthcare professionals with at least 3 months of experience in providing home-visit nursing, medical, or long-term care services (2) Providing care to older adults who meet the inclusion criteria of this study (3) Agreed to participate in the study
Exclusion criteria
Exclusion criteria: There are no specific exclusion criteria for either older adults or coordinators in this study. For coordinators, it is explicitly stated that no exclusion criteria related to educational background or other characteristics are applied.
