Inclusion criteria
Inclusion criteria: Individuals who have received treatment at a Home-Based Medical Care Center Older adults aged 65 years or older Individuals classified as Long-Term Care Grades 3 or 4 Individuals who, after receiving a clear explanation of the study and fully understanding it, voluntarily decide to participate (or whose legal representative decides) and sign the consent form
Exclusion criteria
Exclusion criteria: Individuals expected to be unavailable for follow-up over the course of one year (e.g., patients with a terminal illness and a life expectancy of less than 3 months, individuals planning to relocate to another region) Individuals currently participating in another clinical study that may interfere with participation in this study Individuals with unstable acute conditions for whom inpatient treatment is considered necessary prior to home-based medical care Individuals with severe cognitive impairment or communication difficulties such that study assessments cannot be reliably conducted, even with the assistance of a legal representative
