Inclusion criteria

Inclusion criteria: The Inclusion criteria were as follows: 1. Age = 40 years 2. Diagnosed with acute heart failure by a specialist Criteria: Must meet at least one of the following a-c and criterion d. a. Symptoms of heart failure (breathlessness, fatigue, ankle swelling) b. Signs of heart failure (elevated JVP, pulmonary crackles, peripheral edema) c. Pulmonary congestion or edema on chest X-ray d. Elevated BNP or NT-pro BNP levels (BNP = 100 pg/mL, NT-pro BNP = 300 pg/mL). 3. Classified as frail using at least one tool (TFI or Fried’s Frailty Phenotype) 4. Able to cooperate with functional assessments 5. Provide written informed consent after clearly understanding the purpose and procedures of the stud

Exclusion criteria

Exclusion criteria: The exclusion criteria were as follows: 1. Current enrollment in other programs or planning to participate in similar programs during the intervention period 2. Residing outside Korea and not understanding Korean 3. Diagnosis of dementia with the Global Deterioration Scale stage of 5 or greater 4. Inability to comprehend the study purpose and content.
