Inclusion criteria

Inclusion criteria: The participants were vulnerable older adults aged 65 years and above with chronic diseases. The selection criteria included those who had been diagnosed with and were receiving treatment for two or more major chronic diseases (e.g., hypertension, hyperlipidemia, chronic lower back pain, diabetes mellitus, cerebrovascular disease, and depression) and were receiving livelihood benefits from the government owing to economic difficulties.

Exclusion criteria

Exclusion criteria: Participants with cognitive disorders were excluded.
