Inclusion criteria

Inclusion criteria: Patients with heart failure who: 1) Are 18 years of age and older, and hospitalized for heart failure 2) Are able to respond to the survey and communicate in Korean 3) Owns a smartphone

Exclusion criteria

Exclusion criteria: 1) Having end-stage heart failure(ex. scheduled for heart transplantation, VAD treatment, or hospice care) 2) Having cognitive impairment 3) Planned to be discharged to a facility such as a nursing home 4) Scheduled to be hospitalized for intervention or surgery within 4 weeks (ex. ICD insertion)
