Behavioral : The experimental group uses the Diadic mobile app developed by the researchers for 24 weeks. The control group uses an app developed by the Korean Society of Cardiology.

Inclusion criteria

Inclusion criteria: [Patient] 1. Patients diagnosed with heart failure by a cardiologist and receiving ongoing outpatient follow-up 2. Patients who are alert and able to communicate 3. Patients who understand the purpose of the study and agree to participate in the study [Caregiver] 1. Caregivers who have been involved in the care of a patient with heart failure as the primary caregiver for at least 6 months 2. Caregivers who are alert and able to communicate 3. Caregivers who understand the purpose of this study and agree to participate in the study

Exclusion criteria

Exclusion criteria: [Patient] 1. Patients with a history of stroke, Alzheimer's or vascular dementia, memory impairment, and psychiatric disorders 2. Patients with comorbidities such as chronic kidney failure and end-stage cancer [Caregiver] 1. Caregivers with end-stage cancer, cognitive impairment such as dementia, psychiatric history including depression, or difficulty completing questionnaires
