Inclusion criteria

Inclusion criteria: Inclusion criteria: Patients who get therapeutic drug monitoring of vancomycin during their hospital stay with more than 4 times of vancomycin injection treatment.

Exclusion criteria

Exclusion criteria: patients who are under 18 years of age, under hemodialysis, peritonial dialysis, or continuous renal replacement therapy, and who underwent organ transplant are excluded
