Inclusion criteria

Inclusion criteria: adult patients with acute respiratory failure - 30 minutes after applying High flow nasal cannula(FiO2 0.5, flow 50L) -> PaO2< 150?Hg, PaCO2< 50?Hg

Exclusion criteria

Exclusion criteria: unable to applying prone position(unstable bone fracture, sternotomy, face or chest trauma, etc) unable to applying prone position(agitation, refuse) head trauma, DVT, pacemaker insertion within 15 days spinal or femur or pelvic bone fracture arterial PaCO2 > 50mmHg mean arterial pressure < 65mmHg pregnant women chest tube in anterior thorax acute asthma aggravation cardiogenic pulmonary edema unable to apply prone position due to agitation, patient's refusal intubated patients - clinically detoriated patients - hemodynamically unstable - mental change (GCS < 12) - Sx or sign of respiratory failure
