Inclusion criteria

Inclusion criteria: Patients admittedin the ICU of Asan Medical Center, a university-affiliated, tertiary referral center in Seoul, Korea, over 18 years adults who required mechanical ventilation for 48 hrs is ready to wean and T-piece trial. 1) Partial pressure of arterial oxygen (PaO2) =60 on FiO2 0.5 2) positive end-expiratory Pressure level =6 cmH2O 3) Glasgow Coma Scale scre>13 or Richmond Agitation-Sedation Scale -1 ~ 1 4) Body temperature <38 °C 5) Hct >21% or Hb>7.0g/dl 6) Low dosages of vasopressors or sedatives8 respiratory,

Exclusion criteria

Exclusion criteria: Patients admittedin the ICU of Asan Medical Center, a university-affiliated, tertiary referral center in Seoul, Korea, over 18 years adults who required mechanical ventilation for 48 hrs is ready to wean and T-piece trial. 1) age < 18 2) Pregant 3) Previous history of diaphragm palsy, spine injury and Neuromuscular disease(Guilin Barre disease, Myasthenia gravis etc) 4) Pneumothorax or pneumomediastinitis 5) Large amount pleural effusion or If there is a lung lesion adjacent to the diaphragm 6) Hypothyroidism 7) Uncontrolled arrhythmia or seizure 8) Prolonged Mechanical ventilation over 30days 9) Neuromuscular blocker stopped within 48hours 10) Thoracostomy states 11) Contraindication of EIT: Pacemaker, ACD insertion states, Skin lesion on thorax, over BMI 35
