Inclusion criteria

Inclusion criteria: 1. Adults (19 years old or older) 2. Among the outpatients who were discharged from hospital after an acute period of inpatient treatment, whose primary symptom is stroke and visit organization of pilot project for Korean and Western collaborative. (First or returning outpatients if acute period of inpatient treatment is not required) 3. KCD6 disease codes of stroke are from I60 to I69. 4. The person who has decided to participate in this clinical study voluntarily or the legal representative has signed the consent form.

Exclusion criteria

Exclusion criteria: 1. If a surgery is scheduled within a week after the person was offered a screening of the clinical research, or if the person is visiting for the purpose of hospitalization 2. who is quadriplegia patient 3. If there was a functional impairment that could not lead to independent life due to mental or physical condition before the onset of stroke 4. The person who is participating in other clinical trials (not applicable to those participating in observational studies) 5. The person who is expected to be difficult to comply with the clinical research schedule. 6. The person who is expected to have difficulty in understanding and responding to research questionnaires, such as illiteracy and lack of body and mind 7. Other reasons that researchers find it difficult to participate in the clinical research
