Inclusion criteria

Inclusion criteria: To be eligible for this study, patients must: 1. be hospitalized and have a diagnosis of heart failure 2. be 18 years or older 3. be able to communicate in Korean 4. be able to participate in telephone-based follow-ups

Exclusion criteria

Exclusion criteria: Patients will be excluded from this study if they: 1. have end-stage heart failure (e.g., being on the heart transplant list, being on a ventricular assist device, or receiving hospice care) 2. have a serious complicating comorbidity that can affect prognosis (e.g., renal failure requiring dialysis, malignancy requiring treatment) 3. have cognitive impairment (based on medical records or reports of healthcare professionals)
