Inclusion criteria

Inclusion criteria: All adult patients (aged =18 years)had blood culture result during the study period at Seoul National University Bundang Hospital (a 900-bed tertiary hospital in Seongnam) and Seoul National University Hospital (a 1,300-bed tertiary hospital in Seoul).

Exclusion criteria

Exclusion criteria: Patients who had the following criteria are excluded: (1) polymicrobial bacteremia; (2) immediately transferred to other hospital; (3) no data on treatment outcome; (4) died within 72 hours after index culture; (5) received renal replacement therapy; (6) received vancomycin for less than 72 hours; (7) not receiving vancomycin within 5 days after index culture; (8) vancomycin serum concentration was not checked within the first 5 days of therapy; (9) unavailable index MRSA isolate.
