Inclusion criteria
Inclusion criteria: 1.admitted to Hamamatsu University Hospital for hemodialysis initiation 2.able to provide written informed consent for participation in the study; and 3.independently ambulatory before admission.
Exclusion criteria
Exclusion criteria: 1.unable or unwilling to provide written informed consent; 2.judged to be unable to complete study assessments because of poor general condition; 3.unable to complete questionnaires or physical function assessments because of intellectual disability or language difficulties; or 4.judged by the principal investigator or attending physician to be unsuitable for participation in the study.

| Changes from baseline to 6 months in frailty assessed using the Japanese version of the Cardiovascular Health Study (J-CHS) criteria, physical activity (mean step count on non-dialysis days), physical function (Short Physical Performance Battery, knee extensor strength, handgrip strength, and walking speed), kidney disease-specific quality of life (KDQOL-SF), sleep quality (Pittsburgh Sleep Quality Index), fear of movement (Tampa Scale for Kinesiophobia), and exercise self-efficacy. Adverse events will be recorded throughout the intervention period. | — |
