Inclusion criteria
Inclusion criteria: 1. 18 years or older 2. Hospitalized in a ward targeted by the study 3. Not experiencing delirium at the time of admission 4. Expected survival of more than one month 5. Able to communicate in Japanese 6. Provided written informed consent voluntarily after receiving sufficient explanation and demonstrating adequate understanding of the study
Exclusion criteria
Exclusion criteria: 1. Patients discharged within 24 hours 2. Patients transferred to another ward within 48 hours 3. Patients transferred from another ward 4. Patients with severe psychiatric symptoms, such as impaired consciousness or dementia, whom the principal investigator or co-investigators deem inappropriate for participation in the study 5. Patients participating in another interventional study and judged by the principal investigator or co-investigators to be unsuitable for intervention in this study 6. Any other patients deemed inappropriate for participation in the study by the principal investigator or co-investigators
