Inclusion criteria

Inclusion criteria: 1. Selection criteria for patients themselves Patients who meet all of the following selection criteria shall be eligible for the program. (1) Medical care children (persons) who have a heavy burden due to hospital visits are eligible. i. The researcher will make a comprehensive judgment as to whether the burden of hospital visits is great or not based on the following cases: 1)Wheelchair, buggy, or other means of transportation. ii. 2)Use of home oxygen therapy, tracheostomy, or ventilator for breathing 3)Other: Gastric bandage, NG tube, etc. ii. The definition of "child(ren) in medical care" is based on the definition of "child(ren) for whom it is indispensable to receive constant medical care in order to lead a daily life and social life" in the "Act on Support for Children in Medical Care and Their Families (Act No. 81 of 2021, Ministry of Health, Labor and Welfare)". (2)Must possess a smartphone capable of communication, which is required to operate the TytoCare Medical Assistance Kit. (3) Any age. (4) Any gender. (5) Persons who have obtained written informed consent from a surrogate. 2. Selection criteria for the patient's parents (1)Mothers or fathers who meet the selection criteria for the patient him/herself and who live with and are the main caregivers of the medically cared child(ren) not in conflict with 5.2. Blood relationship is not required. (2)18 years old or older. (3) Any gender. (4)Persons who have given their written informed consent.

Exclusion criteria

Exclusion criteria: 1. Exclusion criteria for patients themselves Patients who are judged by the attending physician to be inappropriate to participate in this study. 2. Exclusion criteria for parents of patients Those who are judged by the attending physician to be inappropriate to participate in this study.
