Inclusion criteria

Inclusion criteria: 1.A physician who gave informed consent on the website 2.Physicians practicing cardiology in Japan at the time of answering the web survey 3.A physician who has provided to 30 or more HF patients in the past month 4.A physician who has provided outpatient care to 10 or more HF patients with eGFR >= 15 and = 15 to < 60 mL/min/1.73 m2 in the past month
