Inclusion criteria
Inclusion criteria: Mature (40-64 years old) patients in stage C of heart failure who were first admitted to the cardiology ward of a community hospital in prefecture A with acute or chronic heart failure, who had not previously received ACP, and who agreed to cooperate in the study.
Exclusion criteria
Exclusion criteria: Patients who have difficulty communicating in Japanese and answering self-administered questionnaires, patients whose physical or mental condition is judged by the physician in charge to be difficult for them to participate in this study, and patients who are in a socially vulnerable position.

| Basic patient attributes include age, sex, presence of a cohabitant, previous and current medical history, New York Heart Association (NYHA) on admission, Left Ventricle Ejection Fraction (LVEF) on admission, Brain Natriuretic Pepteides (BNP). The psychological changes and changes in thoughts about life in care following ACP triggering by nurses are compared pre- and post-intervention on the Subjective Well-Being Scale (SWBS) and the Self-Care Agency Questiomaire (SCAQ). | — |
