Inclusion criteria
Inclusion criteria: (1)Patients who are hospitalized using a diabetes clinical path (guidance content and examinations are scheduled daily within 2 weeks) between the date of study implementation permission and December 31, 2022. (2) Patients who consented to the study (3)Patients scheduled to go to the outpatient department of diabetes, endocrinology and metabolism at the Nihon Keizai University Hospital after discharge. (4) It is possible to communicate through words and exchange conversations with researchers.
Exclusion criteria
Exclusion criteria: (1) Not an adult (2) Diagnosed with dementia (3) Impairment in taking self-management behavior (physical/visual/cognitive function) (4) Diagnosed with stage IV nephropathy or later
