Inclusion criteria

Inclusion criteria: 1. Patients receiving home medical care 2. Patients who fall under the surprise question by the doctor in charge 3. Patients and family members must be at least 20 years of age at the time of consent 4. The research subject or his / her family owns an electronic medium on which ePRO type ACP can be implemented. 5. Patients who have given written consent to participate in this study Patients who meet the above five items, their families, and intervening healthcare professionals are the subjects of this study.

Exclusion criteria

Exclusion criteria: 1. Patients who are unable to create ACP due to impaired consciousness or dementia 2. The official ACP has already been completed 3. Patients who are judged to be ineligible at the discretion of the researcher Exclude patients who fall under any of the above three items, their families, and intervening healthcare professionals
