Inclusion criteria

Inclusion criteria: Approximately 10 heart failure patients are visiting the facility as an outpatient. Approximately 5 nurses who provide ACP support to patients at the facility.

Exclusion criteria

Exclusion criteria: Patients who have indicated that they do not want to discuss their future plans at this time
