Inclusion criteria

Inclusion criteria: Patients who meet all of the following criteria: Age>20 years Require inpatient treatment in the Department of Cardiology at Kasugai Municipal Hospital for acute heart failure or acute exacerbation of chronic heart failure Not enrolled in Long-term Care Insurance at the time of admission

Exclusion criteria

Exclusion criteria: Patients who meet any of the following criteria: Severe hearing impairment or dementia that makes communication difficult Refusal to participate in the questionnaire-based intervention Unsuitable by the principal investigator or a co-investigator
