Inclusion criteria

Inclusion criteria: 1.Age 40 or older at the time of registration 2.Able to measure weight and blood pressure at home 3.Able to own and operate a smart phone 4.Written consent to participate in the research has been obtained from the research subject

Exclusion criteria

Exclusion criteria: Patients whose treatment policy regarding hospitalization, drugs, etc. has been changed during the study period
