Inclusion criteria: Hemodialysis patients over 20 years old

Exclusion criteria

Exclusion criteria: (i) Patients who are judged by their physicians to be inappropriate for inclusion in the program. (ii) Patients who have requested refusal of information use in an opt-out document.
