Inclusion criteria: Inclusion criteria: 1) Patients who participated in the preceding study "Patient Survey on Burdens, Perceptions, Self-care and Treatment Support of Living with Heart Failure in Japan" (UMIN000047055, quantitative research phase) 2) Patients who claimed his/her willingness to participate in this study at the end of the preceding study
