Inclusion criteria
Inclusion criteria: (1) Must be a cancer patient or cancer survivor (2) Must be at least 40 years of age (3) Have a trusted family member or friend (4) Able to communicate in Japanese (5) Consent must be obtained in writing.
Exclusion criteria
Exclusion criteria: (1) Those who are under treatment for depression, anxiety, or other mental illnesses (2) Those who do not have reliable family or friends

| Willingness to implement ACP (1) Willingness to decide on a surrogate decision-maker: "Do you want to decide on a surrogate decision-maker in case you have a serious illness and cannot decide for yourself due to the progression of the illness or other reasons? 2) Willingness to discuss ACP with the surrogate decision-maker: "Would you be willing to discuss specifically what kind of treatment and care you received/would not receive when you were seriously ill, so ill that you could not make decisions, and unable to recover, including the reasons for this? (Select from 6 items on the Likert scale: very much agree, agree, somewhat agree, somewhat disagree, disagree, or not at all agree). | — |
