In the week before the intervention, the amount of physical activity are assessed by a self-rating survey and a wristband-type physical activity tracker. Interview to clarify the goals of participan
s activities of daily living. Goal-oriented rehabilitation support will be provided at least once a week for 3 months.

Inclusion criteria
Inclusion criteria: Those who have survived the acute phase after onset or injury and have stable medical conditions, and use rehabilitation services at least once a week.
Exclusion criteria
Exclusion criteria: It is difficult to self-rating of survey and to manage wristband-type physical activity tracker due to salient decline in cognitive function. It is difficult to continue wearing the wristband-type physical activity tracker due to discomfort or allergic reaction and so on.
