Inclusion criteria

Inclusion criteria: 1.T2DM 2.eGFR 30-<60 3.Those who have voluntarily consented 4.Those who have undergone fundus photography within 12 months before the start of the test

Exclusion criteria

Exclusion criteria: 1.Those who are in the hospital or will be hospitalized 2.Those who are undergoing kidney transplantation or dialysis treatment 3.Those who plan to have a kidney transplant within the study period 4.Those who calculates "Lifestyle-related disease guidance management fee" 5.Those who get diabetes dialysis prevention guidance or nutritional guidance from a public health nurse or dietitian 6.Those who are pregnant or may become pregnant 7.Those who participated in clinical studies or clinical trials within 3 months before consent 8.Those who will move out of Kanazawa at the time of application in this research 9.Those who are judged to be inappropriate as a target by the judgment of a doctor
