Inclusion criteria
Inclusion criteria: Patients at risk of malnutrition (11 or less of MNA-SF or <3.5g/dL of serum albumin level) who regularly visit the hospital three month or more.
Exclusion criteria
Exclusion criteria: Severe conditions of mental illness or other diseases
