Inclusion criteria: 1. Patients with cardiovascular diseases at Tohoku University Hospital 2. 20 years of age or older 3. Women and men 4. Patients or their family caregiver have to have the smart phone

Exclusion criteria

Exclusion criteria: 1. Patients who do not give written informed consent
